Worked as a (live-in)direct support for a dementia patient who is elderly. Tasked with daily assisted living support, medication administration, follow and documenting daily care plans and daily behavioral health assessments on an Electronic Medical Record (EMR). Maintain the home with daily cleaning and cooking. Provide weekly physical activities for client. Take client to medical appointments. Help with mobility and transportation support. Communicate with management regarding any issue encountered. provide hand-off update to relieving care team and nurse.

Provided wheelchair support, assisted with and provided transportation to clients as requested. Maintain and update client condition and activity documents on a regular basis. Inform supervisor promptly of any incidents or irregularities regarding client medical conditions or behavior. Identify defective home equipment and report to supervisor accordingly.
